NICE published its finalised guidance on tirzepatide for managing overweight and obesity (TA1026) in December 2024. Specialist weight management services began offering Mounjaro from March 2025, and NHS England extended access to primary care settings from June 2025. However, access is being introduced very gradually — 250,000 patients over three years, out of 3.4 million who could theoretically qualify. NHS England has acknowledged full rollout could take up to 12 years.
Currently, only Cohort 1 patients are being invited. To qualify you must have a BMI of 40 or above (37.5 for South Asian, Chinese, Middle Eastern, Black African, and African-Caribbean backgrounds) together with at least four qualifying comorbidities — such as type 2 diabetes, hypertension, dyslipidaemia, cardiovascular disease, or obstructive sleep apnoea.
Patients do not apply — eligible individuals are identified through GP records and contacted directly. The NHS has specifically asked people not to contact their GP to request Mounjaro, as local prescribing hubs manage the process.
