A Complete Guide to Mounjaro Dose And How It Changes Over Time

If you are starting Mounjaro or considering it as a weight loss treatment, understanding how the dosing schedule works is one of the most important things you can do before your first injection. Mounjaro (tirzepatide) is not taken at a single fixed dose throughout treatment. Instead, it follows a structured titration schedule, beginning at a low starting dose and increasing gradually over several months. Each step in that process matters, and each one should be guided by a qualified prescriber rather than self-managed based on social media advice or anecdotal experience.

Quick Summary

Mounjaro is available in six doses: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg. Treatment always begins at 2.5 mg for the first four weeks. Each subsequent dose increase happens at four-week intervals and should only be made with clinical oversight. The goal of titration is to find the lowest effective dose with the fewest side effects, not to reach 15 mg as quickly as possible.

What Is Mounjaro and How Does It Work?

Mounjaro (tirzepatide) is a once-weekly injectable prescription medication licensed in the UK for weight management in adults with a BMI of 30 or above, or a BMI of 27 or above in the presence of at least one weight-related health condition. It is a dual GIP/GLP-1 receptor agonist, meaning it activates two separate hormone pathways simultaneously: the glucose-dependent insulinotropic polypeptide (GIP) pathway and the glucagon-like peptide-1 (GLP-1) pathway.

This dual mechanism is what distinguishes Mounjaro from other GLP-1-only medications such as Wegovy (semaglutide). By targeting both pathways, Mounjaro produces a stronger and more sustained reduction in appetite, as well as improvements in blood glucose regulation and insulin sensitivity. Clinical trial data from the SURMOUNT-1 study showed average weight loss of up to 22.5% of body weight at the highest 15 mg dose over 72 weeks, making it the most effective weight loss injection currently licensed in the UK.

The titration schedule exists precisely because of how potent this mechanism is. Starting at a high dose immediately would cause unacceptable gastrointestinal side effects for most patients. Beginning low and increasing gradually allows the body to adapt while still delivering meaningful clinical results.

The Full Mounjaro Dose Schedule

The standard Mounjaro titration schedule follows four-weekly intervals across six dose levels. The schedule below reflects the licensed prescribing guidance for weight management:

  • Weeks 1 to 4: 2.5 mg. The starting dose. This is an introductory dose only and is not expected to produce significant weight loss in isolation. Its purpose is to initiate treatment with minimal side effects and allow the body to begin adapting to the medication.
  • Weeks 5 to 8: 5 mg. The first therapeutic dose increase. Appetite suppression typically becomes more noticeable at this stage, and some patients begin to see measurable changes in weight during this period.
  • Weeks 9 to 12: 7.5 mg. A further increase if the previous dose was well tolerated. Clinical response and side effect profile should be reviewed with a prescriber before proceeding.
  • Weeks 13 to 16: 10 mg. Many patients find this dose delivers a strong clinical response. For some, this may represent the optimal maintenance dose rather than a stepping stone to higher doses.
  • Weeks 17 to 20: 12.5 mg. A higher dose appropriate for patients who have tolerated lower doses well but require additional efficacy to achieve their clinical goals.
  • Week 21 onwards: 15 mg. The maximum licensed dose. Not all patients will reach or need this dose. Those who achieve sufficient weight loss at a lower dose may remain on that dose as their maintenance level indefinitely.

It is important to understand that this schedule represents the maximum rate of dose escalation. Many patients progress more slowly, particularly if they experience side effects at a given dose that require more time to settle before increasing further. Your prescriber will guide each step.

Why Clinical Guidance at Every Dose Step Is Non-Negotiable

One of the most significant risks in the current weight loss medication landscape is the volume of patients attempting to manage medication and dose increases independently, guided by social media content rather than qualified clinical advice. The Mounjaro dose schedule may appear straightforward on paper, but the decision to increase a dose involves a genuine clinical assessment of how the patient is responding to the current level, what side effects are present, and whether the overall treatment plan remains appropriate.

Mohammed Ismail Lakhi, Superintendent Pharmacist and Owner of The Care Pharmacy, makes this point directly:

“We want patients to know that health advice around weight loss medication should come from healthcare professionals and not social media algorithms. Responsible prescribing is about far more than supplying medication. Patients require suitability assessments, ongoing monitoring, education around side effects and long-term support.”

Mohammed Ismail Lakhi, MPharm, MRPharm, Superintendent Pharmacist and Owner, The Care Pharmacy

This is particularly relevant at the higher dose levels. Increasing from 2.5 mg to 5 mg carries a relatively modest incremental risk, as both are low doses and the body is still in the early adaptation phase. Moving from 10 mg to 12.5 mg, or from 12.5 mg to 15 mg, is a different clinical decision entirely. The potential for gastrointestinal side effects, changes in appetite regulation, and interactions with other medications all increase as the dose rises. These are assessments that require a prescriber, not a forum post.

Patients who access Mounjaro through our online clinic have access to ongoing prescriber reviews at every dose stage. If you are experiencing side effects that are preventing you from tolerating a dose increase, or if you are unsure whether your current dose is still appropriate, a consultation with your prescriber is always the right first step.

Side Effects to Be Aware of as Your Dose Increases

Gastrointestinal side effects are the most commonly reported adverse effects associated with Mounjaro, particularly during dose escalation. These include:

  • Nausea, particularly in the 24 to 48 hours following an injection
  • Vomiting
  • Diarrhoea or constipation
  • Reduced appetite extending beyond the expected effect
  • Fatigue and general malaise in the days following injection

For most patients, these symptoms are transient and reduce significantly once the body has adjusted to a new dose level. They are most pronounced during the first one to two weeks after each increase. Practical measures that can help include:

  • Eating smaller, more frequent meals rather than large portions
  • Avoiding high-fat or heavily processed foods, particularly in the days following injection
  • Staying well hydrated throughout the week
  • Injecting on the same day each week to maintain a consistent hormonal pattern
  • Avoiding lying down immediately after eating

If side effects are severe, persistent beyond two to three weeks at a given dose, or affecting your ability to eat or drink adequately, contact your prescriber before proceeding with the next scheduled dose increase. In some cases, remaining at the current dose for an additional four weeks, or temporarily reducing the dose, is the clinically appropriate response rather than pushing through.

Exercise to support weight loss

Does Everyone Need to Reach 15 mg?

No. The 15 mg dose represents the maximum licensed dose, not a universal target. The clinical objective of Mounjaro treatment is to achieve meaningful, sustainable weight loss with the best possible tolerability profile. For many patients, this goal is met at 10 mg or 12.5 mg, and there is no clinical rationale to increase further if adequate progress is being made and the current dose is well tolerated.

The published 22.5% average weight loss figure comes from patients on the 15 mg dose in SURMOUNT-1. Patients on lower doses in the same trial still achieved clinically significant weight loss:

  • 5 mg dose: average weight loss of approximately 16% of body weight over 72 weeks
  • 10 mg dose: average weight loss of approximately 21% of body weight over 72 weeks
  • 15 mg dose: average weight loss of approximately 22.5% of body weight over 72 weeks

These are averages across trial populations. Individual results will vary. Your prescriber will help you identify the dose level that represents the best clinical outcome for you specifically, taking your weight loss progress, tolerability, and overall health into account.

Frequently Asked Questions

Specific questions about the Mounjaro dose schedule that our prescribing team regularly receives from patients.

Can I stay on 2.5 mg indefinitely if it is working well enough for me?

The 2.5 mg dose is classified as a starting dose in the licensed prescribing guidance, not a therapeutic maintenance dose. It is not expected to produce clinically significant weight loss at this level for most patients, and remaining on it long term without escalation is unlikely to deliver meaningful outcomes. If you are tolerating it very well but concerned about side effects at higher doses, speak with your prescriber about a slower titration schedule rather than remaining at the starting dose indefinitely.

What is the clinical rationale for the four-week intervals between dose increases?

Tirzepatide has a half-life of approximately five days, and steady-state plasma concentrations are typically reached within two to four weeks at a given dose. The four-week interval allows steady-state concentrations to be fully established and gives the gastrointestinal system sufficient time to adapt before the dose is increased. Accelerating this schedule increases the risk of significant nausea and vomiting without necessarily improving efficacy outcomes.

If I have previously taken semaglutide, do I still need to begin Mounjaro at the 2.5 mg starting dose?

Yes. Prior GLP-1 experience does not change the required starting dose for Mounjaro. Tirzepatide has a different receptor profile to semaglutide, and the body’s response cannot be directly predicted from prior GLP-1 tolerance alone. The licensed prescribing guidance requires initiation at 2.5 mg regardless of previous treatment history. Your prescriber may be able to progress through the titration schedule more confidently if your prior GLP-1 tolerance was good, but the starting point remains the same.

Is it safe to increase my dose sooner than four weeks if I am not experiencing any side effects?

No. The absence of side effects at a given dose does not mean the body is ready for an earlier increase. Steady-state plasma concentration of tirzepatide is not reached until several weeks into treatment at any dose level. Increasing before this point means you are escalating before the full pharmacological effect of the current dose has been established, which does not serve the purpose of titration. Any change to the prescribed schedule should be discussed with your prescriber, not decided independently.

If I need to pause treatment for a number of weeks, do I have to restart from 2.5 mg?

This depends on the length of the break and the dose you were on when treatment was paused. A short interruption of one to two weeks at a stable dose may not require restarting from the beginning. Longer breaks, particularly at higher dose levels, can result in a reduction in tolerance, meaning restarting at the previously established dose carries a greater risk of significant side effects. Your prescriber should always be consulted before resuming after any break of more than two consecutive doses.

Can the Mounjaro dose be reduced rather than stopped entirely if I am struggling with side effects?

Yes, and in many cases a temporary dose reduction is a more appropriate clinical response than discontinuation. If side effects at a higher dose are persistent and affecting quality of life, reducing to the previously tolerated dose for a further four weeks before attempting the increase again is a recognised prescribing approach. This should always be done in consultation with your prescriber rather than through self-adjustment, as the clinical picture needs to be reviewed as a whole before any change is made.

Does the injection site affect how quickly the dose is absorbed, and does this vary between dose levels?

The recommended injection sites for Mounjaro are the abdomen, upper thigh, or upper arm. Absorption rates can vary slightly by site, with the abdomen generally offering the most consistent absorption. While the clinical significance of site variation is modest for most patients, it is advisable to rotate injection sites consistently rather than always injecting in the same location, as this reduces the risk of localised tissue reactions such as lipohypertrophy. The same rotation principle applies across all dose levels.

Getting Started with Mounjaro

Mounjaro is one of the most clinically effective weight loss treatments currently available in the UK, but its effectiveness is directly tied to how well it is prescribed, monitored, and managed over time. The dose schedule is not a formality. It is a clinically designed framework that protects patients from the risks of too-rapid escalation while maximising the opportunity for meaningful, sustained weight loss.

Accessing Mounjaro through a GPhC-registered online pharmacy means every step of that process, from your initial suitability assessment to every subsequent dose review, is overseen by a qualified prescriber. This is not an inconvenience. It is the mechanism that keeps patients safe and treatment effective.
