What happens to your current dose when you switch?

A note on treatment gaps

If your previous provider experienced stock shortages and you have already missed one or more doses, our prescriber will factor this into your assessment. Can I switch without having to restart on the lowest dose?

For most patients who are established and tolerating their current dose well, it is not necessary to restart from the lowest dose when switching providers. The medication itself does not change. Our prescriber will review your current dose, tolerance history, and any side effects before confirming the prescription. If everything looks clinically appropriate, treatment continues at your existing dose. If there are reasons to suggest a different approach — such as a treatment gap or new health information — our prescriber will discuss this with you.

Will there be a gap in my medication supply when switching?

The time between your consultation and receiving your first order from The Care Pharmacy is typically 24–48 hours for working day orders approved before 2pm. To avoid a gap, it is worth beginning your consultation with us while you still have a small supply remaining with your current provider. Do not stop your current medication before your new prescription has been confirmed. If you are already in a gap due to stock issues elsewhere, note the date of your last dose in your consultation — this information helps our prescriber make the right prescribing decision for you.

Do I need a new prescription to switch providers?

Yes. A prescription from your previous provider cannot be transferred to The Care Pharmacy for dispensing — each pharmacy issues its own prescriptions following a clinical assessment. You will need to complete a new consultation with us. This is a legal requirement, not a commercial one, and it exists to protect you: the prescriber issuing your prescription is accountable for that clinical decision. The consultation is free and is included in your treatment price.

Does switching cost anything extra?

No. The clinical consultation is included in the price of your treatment — there is no separate fee to switch or to register as a new patient. The price you see on our treatment pages is your total monthly cost, including consultation, pen needles, sharps disposal, and free delivery. The only cost is your normal monthly treatment price.

What if I am mid-titration when I switch?

Being mid-titration is not a barrier to switching. When you complete your consultation, you will be asked to provide your current dose and titration history. Our prescriber will assess whether it is appropriate to continue at your current dose, move to the next scheduled dose, or — in specific clinical circumstances — temporarily adjust. The most common outcome for patients mid-titration is continuation at their current dose, with the next titration step happening at The Care Pharmacy on the standard schedule.
