There is no single right answer for every patient in Cardiff. If you are clinically eligible and can wait, the NHS route costs nothing at the point of use, which matters a great deal to many people. If waiting is not realistic for you, or your GP practice cannot refer you into a specialist service, a private clinic or a regulated online pharmacy becomes the more practical option. Between those two, the choice often comes down to whether you value face-to-face contact enough to justify a higher cost, or whether a fully remote service that still involves a genuine clinical assessment and named prescriber suits your circumstances better. Whichever you choose, the questions worth asking are the same: who is assessing me, what happens if I have side effects, and what ongoing support is included.
Mounjaro is a weekly injection built on tirzepatide, working through GIP and GLP-1 pathways together to reduce appetite and extend fullness. Licensed for adults meeting specific BMI criteria, used alongside diet and activity changes.
Wegovy uses semaglutide alone, also weekly, licensed for adults meeting relevant thresholds. Prefer to skip injections? The Wegovy pill offers semaglutide as a tablet, going through the same assessment process.
We also stock Saxenda, a daily injectable GLP-1 medicine, Nevolat, another daily injectable option, and orlistat, which reduces fat absorption in the gut rather than affecting appetite. All Prescription Only, all assessed individually.
None of these come without possible side effects, and none is suitable for every patient. A genuine look at your medical history and current medications always comes first. Eligibility criteria differ slightly across these options, which is exactly why one-size-fits-all doesn’t work here.
Both Mounjaro and Wegovy are given as a once-weekly subcutaneous injection, usually into the abdomen, thigh, or upper arm, on the same day each week. Neither medicine is started at the full maintenance dose. Mounjaro typically begins at 2.5mg weekly for the first four weeks, a dose intended to help the body adjust rather than to drive weight loss, before increasing in stages, such as to 5mg, 7.5mg, 10mg, 12.5mg, and up to 15mg, at intervals of around four weeks, as tolerated. Wegovy follows a similar principle, generally starting at 0.25mg weekly and increasing every four weeks through 0.5mg, 1mg, and 1.7mg, up to a maintenance dose of 2.4mg. The exact pace of titration is always decided by your prescriber based on how you respond, and some patients remain on a lower dose for longer if side effects require it.
It is natural to want a clear answer on how much weight you might lose, but the honest answer is that results vary considerably between individuals. Clinical trials of these medicines have generally shown meaningful average weight loss over sustained treatment, typically building gradually over many months rather than appearing in the first few weeks. How much any one person loses depends on their starting weight, dose, how consistently the medicine is taken, and the extent to which dietary and activity changes are also made. We would be cautious of any provider promising a specific number, since no clinician can guarantee an individual outcome in advance.
None of these medicines are designed to work in isolation. Mounjaro, Wegovy, Saxenda, Nevolat, and orlistat are all licensed to be used alongside a reduced-calorie diet and increased physical activity, not as a replacement for either. In practice, this means paying attention to protein intake to help preserve muscle mass as weight is lost, staying well hydrated, and building activity levels up gradually rather than all at once. Many patients in Cardiff also find it helpful to keep a simple record of meals and how they are feeling, which gives useful information to share at follow-up reviews.
If your BMI sits just below the usual private prescribing thresholds, this does not necessarily mean medication is off the table indefinitely. A prescriber may discuss interim steps, such as structured lifestyle changes, or may reassess you again after a period of time. Where a weight-related health condition is present, such as type 2 diabetes, high blood pressure, or sleep apnoea, this can sometimes bring the relevant threshold down, which is why a full and honest conversation about your health during consultation matters more than trying to estimate your own eligibility in advance.
As a general guide, private prescribers look for a BMI of 30 or above, or 27 or above with a weight-related condition such as type 2 diabetes alongside it. The exact number varies a little by medicine, which is why individual assessment matters more than a general rule.
