Foundayo vs orlistat: patient guide
Foundayo and orlistat are both taken by mouth, and that is roughly where the similarity ends. Foundayo works on appetite through GLP-1 receptors. Orlistat stops some of the fat in your food being absorbed. At The Care Pharmacy we get asked to compare them a lot, so this guide sets out how they differ in eligibility, routine, eating, side effects, other medicines and cost, so you can have a proper conversation with our prescriber rather than picking a name.
Foundayo contains orforglipron. It is a once daily tablet licensed in the UK for weight management in eligible adults, alongside a reduced calorie diet and more physical activity. The MHRA announced UK authorisation in August 2026. It works on the GLP-1 receptors involved in appetite, so you tend to feel full sooner and think about food less.
Orlistat is a completely different kind of medicine. It blocks the enzymes that digest fat, so around a third of the fat you eat passes through unabsorbed. Everything about it, from when you take it to the side effects you get, is tied to your meals.
In this guide we compare Foundayo with prescription orlistat 120mg, using the Xenical product information and the Foundayo product information. Lower strength pharmacy orlistat has its own rules and is not the same product.
Both need a realistic eating and activity plan. Neither guarantees a particular amount of weight loss. The useful questions are:

Foundayo’s weight management licence covers adults with a starting BMI of 30 or more, or 27 or more with a weight related health condition. Prescription Xenical uses 30 or more, or 28 or more with risk factors. The thresholds are close but not identical, and they are only the start. Our prescriber also has to consider contraindications, interactions and whether the medicine is right for you as a person.

Pregnancy, breastfeeding, digestive conditions and other health problems can rule an option out. Please do not decide on a medicine first and then leave out the detail that would make it unsuitable. One tablet a day, following the prescribed schedule. The UK tablet does not need the fasting routine that some other oral GLP-1 medicines require, so do not carry over instructions from a different product. Our prescriber will explain your starting strength and when it increases. Follow the prescription and the leaflet in the box, not a dose you have seen in a trial summary.
One capsule with each main meal that contains fat: immediately before, during, or up to an hour after. If you skip a meal, or the meal has no fat in it, you skip the capsule.
Orlistat asks you to pay attention to every meal. Foundayo asks for a consistent daily habit and a clear plan for dose changes and review. If you work shifts or eat irregularly, tell us. And with either medicine, do not invent a catch up schedule for missed doses or take extra to make up for a richer meal.
Your appetite will change, and food may feel less comfortable in larger amounts. That does not mean eating as little as possible is the goal. We will help you keep your nutrition adequate while the medicine does its job.
The bowel effects depend on how much fat you eat. A very fatty meal makes oily stools, urgency and leakage more likely. The product information recommends spreading your fat intake evenly across your three main meals. Do not respond by cutting fat out completely. Fat is part of a normal diet, and the aim is balance.
Orlistat also reduces absorption of fat soluble vitamins (A, D, E and K). We may recommend a multivitamin and explain when to take it so it is not blocked too. Ask before adding any supplement yourself.

Results depend on the medicine, the dose, how long you take it, the support around it and you. Trials of different medicines use different people and different methods. A headline percentage from a study is not a prediction of what will happen to you, so we will not quote one at you.
What we will do is agree a reasonable goal and a review plan. That covers weight change, how you are tolerating the medicine, your health measures, and whether you can keep to the eating and activity plan.
Orlistat has a built in checkpoint: the Xenical product information says to stop after 12 weeks if you have not lost at least 5% of your starting weight. Foundayo has its own review plan, agreed with you. If progress is disappointing on either, talk to us. Do not increase the dose early, combine medicines, or buy something else online.
Nausea, diarrhoea, vomiting and constipation are common, especially early on and after dose increases. Persistent vomiting or diarrhoea can dehydrate you and strain your kidneys. Contact us if you cannot keep fluids down or feel properly unwell.
Severe, persistent stomach pain, especially spreading to your back, needs urgent assessment. Pancreatitis is a recognised risk with GLP-1 medicines. Stop Foundayo and seek urgent medical help. Do not put severe pain down to your body adjusting.
Oily stools, urgency and wind with discharge are common and can be embarrassing. Tell us if they are interfering with work, travel or daily life. Severe or persistent abdominal symptoms still need checking rather than being written off as expected.
Orlistat is unsuitable if you have chronic malabsorption syndrome or cholestasis, and must not be used while breastfeeding. Kidney problems and other conditions need extra thought.
Call 999 for severe allergic symptoms such as difficulty breathing or swelling of the mouth or throat. Keep the packaging so the medical team knows exactly what you have taken.
Interactions are one of the biggest differences between these two medicines, because they affect the body in different ways. Give us the full list: prescriptions, occasional medicines, supplements and anything bought online.
Foundayo has specific interactions involving how medicines are transported and broken down, and it slows stomach emptying. If you have a procedure coming up that needs anaesthesia or deep sedation, the team doing it must know you are on a GLP-1 medicine.
Foundayo can make the contraceptive pill less effective. The product information says to switch to a non oral method, or add a barrier method, for 30 days after starting and for 30 days after every dose increase. We will talk this through before you begin.
Orlistat can affect ciclosporin, warfarin and other anticoagulants, levothyroxine, and some epilepsy and HIV medicines. That may mean extra monitoring, spacing doses apart, or a different treatment. Never solve an interaction by stopping an essential medicine yourself.
Severe diarrhoea on orlistat can also make the pill less reliable, so extra precautions may be needed then. That is a different issue from Foundayo’s 30 day rule. Ask us for advice that matches your actual contraceptive.
Neither medicine should carry on into pregnancy without review. Foundayo must not be used in pregnancy or while breastfeeding, and its product information says to stop at least three weeks before trying to conceive. If pregnancy is possible or planned, tell us before starting either medicine, and we will agree contraception, a stopping plan and follow up that fits you.
Do not combine Foundayo and orlistat, and do not switch between them, without a plan from our prescriber. The fact that they work differently does not mean taking both is safe or more effective.

Compare the full cost of the exact prescription and everything that comes with it. A medicine price on its own can leave out the consultation, delivery, monitoring and follow up. Ask what later stages of treatment are likely to cost, not just the first month.

Not without an explicit plan from a prescriber. Different mechanisms do not make the combination safe or more effective. If one medicine is not helping enough, review it with us rather than adding a second product yourself.
